Case Management

Case management will be facilitated by a licensed, certified or credentialed healthcare professional where the medical diagnosis and treatment plan will be outlined for the patient and any concerns they may have resolved.  This may include referrals for second opinion or consideration of other treatment options in response to the patient’s expressed desire.  Review charts, assuring referrals have been made that needed to be made, that referring doctor as sent back appropriate report covering areas indicated in referral, make certain chart is maintained in clear and legible basis, all tests ordered, test results reviewed and signed and maintaining adequate oversight of treatment, assuring all pertinent information is provided to the attending physician or medical provider until the patient is discharged.     

    

Patient with Chronic Illnesses

As a part of our intake documentation, we seek to document whether the patient suffers from any persistent or lingering medical condition. In examination, our certified medical staff seeks further information and detail by interviewing the patient concerning the information contained in the intake documentation.  The medical provider takes into account the information and considers the report in context of what findings result from the physical examination and other diagnostic procedures.  Together, a comprehensive insight into the factors contributing to the patient’s conditions should arise.  We will assess the factors intrinsic to the patient’s condition, e.g., environment, lifestyle and develop treatment plan to intervene.  Where we may be ineffective in managing the issue, we will refer the patient where they receive specialized care for their condition.

Informing Patient

Upon a completed diagnostic, we provide our patients with clear explanations of our findings.  In cases where there are indications of grave medical issues, we advise patients they are free to seek a second opinion. We also encourage the same when a patient or their significant other questions our diagnosis or treatment recommendation.  In instances where the patient has no referral choice of their own, we will provide a list of medical providers who also provide PPP services to follow-up. 

 

In situations where a patient may not fully comprehend a communication of a medical diagnosis, with an authorization for a release of their medical information, we will share information of the findings with their significant other.  This is especially relevant in situations where there is an element of fear or the implications of the medical treatment are of grave proportion.

 

 

Prevention of Disability and Disease

We focuse our approach on educating and persuading individuals to avail themselves of medical services to impact the incidence of disease and disability.  We capitalize on our experience in communicating effectively with uninsured low-income and indigent persons.   

In summary, our preventative health care plan is:

 

v     We are diligent in maintaining an adequate stock and access to resources for immunizations, treatments and other preventative services. 

 

v     We maximize the outcomes of our outreach efforts by addressing preventative health care issues of factors that effect large numbers of persons, e.g. the weather.

 

v     We are mindful of how neglect and other factors impact persons with existing injuries or disabilities and their needs for preventative health services.

 

v     We capitalize on weather consciousness to cultivate preventative health care participations. We will integrate a computerized database and tracking system to be proactive in our follow-up of patients to schedule preventative health care services.

 

v     We consistently monitor our calendaring system to identify patients needing to be contacted for a review of their need for preventative health services and to track of follow-up and treatment of patients.

 

 v     It is our belief that persons who have no medical coverage’s are sensitive to the subject of their vested interests in preventative health care.  We also believe they would be motivated to avail themselves of preventative care when made aware a lack of medical insurance would not prevent access to medical services.   As a result, we believe our outreach would not necessarily depend on persuading individuals, but to inspire their taking advantage of PPP by emphasizing the value it represents to one of their pre-eminent needs.
